The whole pack is for UNPB and the shoemeshes contain a modified Feet mesh.  Thus the open shoes do not work with the normal UNP Textures.......

 

Any clue about where to get that eyepatch and weapon?

You need to buy Gatcha coins (VERY expensive) and use them in the capsule machine. It may take a few tries. Or just search "eyepatch" in the console.

Anyone know where the location for the set is? I leveled up my blacksmithing to max and i couldnt find it in the forge so i have no clue where it is

 

 

From what i can see in the .esp it seems like you have to use the console to get the pieces...

 

sry i can´t be of more help...    i never use the plugins that comes along armor/clothing mods

nah, it is like an effect, like wear and tear effects... sometimes it is normal but very often it is as it is... even when i am wearing an armbinder it sometimes appears...

I know there is a Shame setting in Devious Device - More Devious Quest, don't know if it's the same as your problem. But if it's an active effect you can check what mod gives it if you type help "device status" in your console.

More Devious Quest has similar "shame" effect. Can be disabled in mcm menu.
