How is this triggered in an existing game where you've already joined either the Legion or the Stormcloaks and already in partway of the Civil War?

ToH merged has two add-ons for Dawnguard and Dragonborn included. It's not standalone.

Serpent must give Civil Whore quest that activates one of them. Also in the MCM there should be Quest page where you can activate these quests.

im on the quest where the character helps onmund study by taking her clothes off. After he fails the first time he says to wait 3 hours, after which he would try again. I've waited over 20 hours and theres no dialogue option to advance the quest yet. Any ideas?
