I have no idea how to bring this into a game engine because the only way I see to make this is a vertex animation and they are not supported in the most game engines. 

A other way would be morphs like the weight morph in skyrim, but this moves the vertices linear and not over the "glans-bulge".

There is one person called Eria who made the custom model/skeleton as mod.

Btw in the picture is everything modifiend to my liking. I guess there is not armor supported etc. so everything has to be made custom. 

 

Yea and skin of Lightforged is made by me from original WoW testures via PS. It is raw version tho and not high quality. But still looks cool.
