Couldn't get anything from Madanach so I setstaged. Got the quest to talk to Falk which worked. Told to talk to Gen Tullius but he has no dialogue options, just the vanilla stuff.

I'm trying to figure out what stucks you,at which stage are you on afs5oldgods1 when you have finished the"truce"? if is 65 you have to meet katalia so don't go to Samarie but straight to River Source and seststage 70 then speak with Katalia,follow the scenes with Avatars and after those katalia will folllow you to Samarie in her den.,About problems with Madanach's followers probably they are connected with "escape"it happened to me too but with a mod found on Nexus called "are you there?"it possible to solve a lot of issues.I don't know if on this side is allowed enter in particulars otherwise my apologies but I recommend it.

River Source Tree is where you meet Ariovistus at the beginning for hired,in the previous post I missed that there is another stage at Lore Tree before to go to Samarie

Found Katalia, talked and she got fucked by a big chaurus. Told to go to Lore Grove. (no map marker).

Been in the grove which was still burning, nothing happened so came out again. Now outside Lore Grove with Katalia who has no new dialogue. afs5oldgods1 is at stage 90.

Don't enter in the grove but near a cutted tree outside and sestage at 100,a new dialogue will appear and a new Avatar will make sex with Katalia after this you'ld go  to Samarie,use the spell to teleport in the Lost valley and Katalia will follow you.

maybe now there is a problem,after speaking with Samarie,she will thank you for your efforts and she wants to know Katalia better and the diary will tell to follow the chat between the two girls but Katalia is stuck outside,you can wait for days but nothing so call her by  "are you there?" ,she appears but no chatting so setstage 140 and you could finish this enough bugged quest that we hope Delzaron will fix

I added a schortcut after Katalia and chaurus scene... the lore grove is not necessary... maybe you used setstage, and got it...

Looked inside your mod for educational reasons and found your "Madanach's bug".

It is actually kind of hilarious how you have so much energy to make those incredible huge mods

and no energy at all to finish them to playable state.

So problem is actually very simple. You swapped original fragments from Madanach's escape quest

on his farewell dialog with your own scripts (see screenshots below) that do the same thing and then start your quest.

And then never compiled them. So those scripts do not exist when game executes and he does nothing.

Compile both of them and everything will work as intended.
