edit: Figured out how to download. Apparently even with an account, the site requires that you install some spyware download manager program in order to download certain files (presumably above a given size threshold). I installed the spyware download manager on a VM and managed to download the mod using the manager.

 

Attached here in case anyone else wants it later.

I did the standard bsa repackage, nif and texture full optimization with Cathedral Asset Optimizer, resaving the ESP to version 44, and even replacing the body nifs with a working SSE CBBE/UNP and all of these still resulted in instant CTD when spawning the follower onto the map. I've managed to port all of the mods I've tried so far except this one. The meshes and textures are loading fine when opened in Nifskope.

Weird never seen this. She has two pairs of eyes in her head. Also, she is using some sort of unique eye-meshes which I have not seen before.

I think this is it...   It has a 'fomod' for some reason-  I deleted it from the rar because it didn't do anything, basically was just a title menu.  Also, the names are all just boxes in-game, it's not in English.

Yes, I noticed that earlier as well. When I tried to regenerate Facegen using CK, the resulting nif not only had two pairs of eyes, but also two noses and mouths too. (the texture got offset somehow). This CK mesh does not cause CTDs when loaded into SE, but looks exactly as it shows up in Nifskope.

When I tried to copy the old branches of the optimized nif into the CK nif replacing the parts the CK messed up, every single option (brows, head, hair) results in a CTD.
