I unzipped DH ENB after like this

 

 

You use the Default file, and then overwrite it with one of the other files of you choice.

All files seem to contain different Presets as you can probably tell.

That is caused by a wrong load order, probably you have Bijin Warmaidens with/or EEO.

It's in Vanilla Skyrim, you will either need Racemenu or a custom plugin to enable all tattoos etc for use on all races etc. I think it's woodelf by default.
