That's strange. I don't use MO2, so I couldn't tell you if everything is in order. Try forcing it with this: 

 

SetStage _CS_House 96

 

Yes, you can remove those facial piercings. During Pet Walk, go to Simone, Wolfgang's guard at Drumlin Diner. She can put in sets or remove them all.

Lindstrom can swap them about as well, doesn't he? Once you ask him for a workbench permission? 
Also I think in my recent playthrough he gave his speech and the outfit the second time I selected the Pet Walk, not straight away.

I am having the same exact issue you are describing, did you manage to solve it?

It’s during the second playthrough of Pet Walk he’ll gives the Litheria Outfit. You’ll hear a different of dialogue.

I absolutely love this mod.  I waited a while before starting it up and loved that when I revisited my settlements during Pet Walk they told me to take a hike for being a raider slave.

 

One thing I would love to see is a way to go back to the slavers after taking Gristle's escape route.  Like, after escaping run off and try Nuka Ride or mess around with Harassment and consequences from Violate or doing whateverthefuck.  But, ultimately, it all proves to be too much.  She realizes that she is broken and incapable of surviving the wasteland - that she was better off as a raider slave so she goes crawling back to Corvega to put on another collar for the permanent slave route.

Forgot to mention as well, during The Scientist Quest, Nora does not get kidnapped by the Minuteman (General Becker) for some reason to start the quest The Minutemen. I had to Setstage _CS_Minutemen 100 to activate it. So it would be right by SetStage _CS_Duff 130 for The Scientist quest. Then after completing The Minutemen quest I had to paste SetStage _CS_Duff 140 to advance the quest because I could not interact with Dr. Duff to give her the beast sample.

 

This is when you do the quest the first time when you ask Lindstrom for an assignment. I have not tested if it happens during the second time.
