IIRC If it uses a custom body with a mesh that's too radically different from the male body you gotta make a new race for the femboy and edit every armor piece to use different art for the race the NPC will use. It can be done pretty easily in TES5Edit without the risk of constant crashing like the creation kit, which is good because you need to edit a lot of armor pieces XD.
