If you have an outfit that IS NOT revealing → it has safe keywords, NPCs won't comment. If you have an outfit that IS revealing → it has (slightly) unsafe keywords, NPC may or may not comment. But, if you have an outfit that IS NOT revealing, but you CAN MAKE IT revealing by modifying it on armor bench (swapping the mesh - for example: zipped to unzipped), that won't work, I'm afraid. The only such mod that comes to mind is of that Modular Vault Suit. It features multiple meshes of the vault suit. When you first create it, it will be zipped, and you can modify it on armor bench to be fully unzipped. IF I recall correctly, I'm not running that mod. I didn't look too much into it as it'd be a lot of work. I don't even know if it's possible to achieve it. It's not SAKR and its keywords we're talking about now - it's the RobCo Patcher itself. I'm afraid your only options are either a) choose keywords by balancing it out as much as possible or b) just pick one target mesh that you're most frequently using and stick to it. I won't be releasing this tool for Skyrim, as I don't play it. I hope this helps!
