A Strange Mesh Glitch

Why is his head missing a mesh? What did I do?

The sex animations are literally just this right now.

Red box with exclamation point = missing meshes find the missing meshes. Check whatever race you are using and make sure the meshes are placed in the correct spot and make sure you have all of the required mods if it requires anything else. Reread the threads first post where you downloaded it from and look at requirements and see if there are any known conflicts. Or try a different mod if that one is broken/old.

It's not my character's head, it's Doc Mitchel's.

Then there is something altering him as his body is in a BSA lose files or mods both load after the BSA's load so there is something that is overwriting him or conflicting. You might have to fire up FNVedit and look through the fallout NV .esp and see if there is anything altering it.

Can you walk me through how to?

1. Open FNVEdit and click OK. Make sure that all the mods are checked.
2. Expand FalloutNV.esm tree.
3. Scroll down to None Playable Characters and expand that.
4. Look for Doc Mitchell and select it.
5. On the top it will list what other mods alter this NPC.
6. Start checking each mod that may alter Doc Mitchell.

If you installed some NPC overhaul replacer, make sure you've installed all the files.

I'm going to take a wild stab at this and say it's "New Vegas Redesigned II". 

You don't need both 2 and 3 versions, and if I remember correctly, either one or both version called for "Fallout Character Overhaul" assets which you may or may not have added. FCO gave Doc Mitchell a new head mesh, whereas NVR3 does not change him. 

So, you may have loaded the assets, and then removed them at some point. (?) 

Either way, just get rid of NVR II.

Uninstalled NVR 2, and Doc Mitchell still looks deformed and my character still has no sex animations.

Where to begin...

 

Besides what others have said, sort with Loot, its obvious its not sorted correctly, sexout-amara.esp is no longer needed integrated with latest stable SexoutNG (built in), read the directions. Some of the older Sexout mods may not work correctly with the new stable build of SexoutNG, check first. Disable all your mods and then add few at a time until problem reappears, then work back to find the one that is the problem. Your also missing a merged patch winch is pretty much required. Don't load 400 mods up all at once an expect them to work, recipe for failure. You must add few at time and test otherwise troubleshooting is impossible.

 

From your image it does not look like DM is the issue but as myuhinny said "missing mesh", the mesh I see missing is your characters hair. Could also be any other object they are wearing like necklace, earrings etc.

My character is fine, however she will not perform sex animations.

YOU need to go back over ALL your mods and make sure all requirements are met. even reinstall some that change npcs. pretty simple just you need to do it. back track what mods you have recently installed ( i'm sure this isn't the first time you have seen doc.)

I fixed the mesh problem , however my character still isn't copulating, but rather just stands there as Doc Mitchel is fucking her while she's standing still.
