Incompatible/incorrect body textures for the body being used. If you use a body texture that is not compatible with a body the nipples will be in the wrong place.

I've seen a problem before where people had no texture and the body had no nipples at all...or anything else.

 

^post is right, look for a texture that fits. Whatever you're using. (Why people like those sized breasts, is beyond comprehension).
