My eyes aren't the best, so I'm guessing that your Sim is missing a penis. The guy looks kinda blurry, too, but that's probably just due to image resolution or my screen.

 

Anyway, if a Sim is missing a body part, click on that Sim while no sex is happening. Select "Wicked">"Personalization">"Body Selector" and then either "All Sims" to set a new default value for all Sims like the selected one (in this case: all males that have or should have a penis), or "This Sim", if it is a Sim-specific issue or you just want to change that one Sim. In the menu that comes up, check on both the soft and hard penis categories.

If an option is set to a body part created by "Maxis" (creators of TS4, who didn't include a penis), change it to something else (you should at least have WW's default available).

If it is set to something other than WW's default, write down the names of both the body part and creator for later. Then change the selection to WW's default.

If WW's default doesn't work, don't change anything.

Now quit the game and take a look inside your Mods folder.

If you wrote down something earlier (or memorized it any other way), this is the moment to search for those names. The file(s) you will find, belong(s) to broken CC and should be removed from your Mods folder (if updating is not an option, of course).

If WW's default gave you trouble, look for any file names containing something like "default" or "DR" (short for "default replacement"). Since WW comes with a set of "default" body parts already, you cannot have a second default penis model, nude top or other body part. Remove the file(s) from your Mods folder, and everything should be fine again.

if it is about the blurred Sim, you did not read the instructions properly  go to your mods folder and remove the TURBODRIVER_WickedWhims_StreamerMode.package
