Did you ask whomever published the pic? It looks vaguely similar to a person I follow on Flickr. I found a tutorial on how to add my own tats several months ago. Once you do it a couple of times it gets kinda easy. Requires Mudbox and Gimp or PhotoShop with the dds plugin. Here is one I did for FO4-
