It has nothing to do with the ECO EP but it's caused by outdated extra tongues and/or penis rigs. They can sometimes be hidden cc in older downloaded male sims along with other cc that might even be duplicates of what you already have. However major game updates like ECO and Star Wars broke many mods and they need to be hunted down and either updated or removed. And many don't delete their localthumbcache.package file in their TS4 folder after adding, removing, or updating mods or game.
