Hostile NPCs Are Not Fighting Back

I've been having this issue with skyrim off and on for a while now, at some point in a skyrim play through the hostile npcs( bandits,draugr,ect.) will just not attack, no matter what I do and it makes the game boring. Please I desperately need a solution to the problem as its kinda game breaking, a fix will be greatly appreciated.
