I visited the mage and requested to add seven tattoos, but only ended up getting three or four new tattoos added. Is it possible that a heavily modded game caused this issue or is it just a random selection that happened to pick the same place repeatedly? Could it be that the area has reached its limit for tattoos?" exceeded the limit for tattoos in this area?won't place?

I doubt that a heavy setup is a problem anymore nowadays with SE and modern hardware. 
Ensure you have enough free overlay slots, dont forget that things like dirt and cum also take up slots for example. You can increase your overlays slots in race menu's skee64.ini. Rape tats has a explanation in its tips section if you need it.
Look in the slavetats mcm to see what tattoo's you got and its not just in a place that you didnt look at like the foot. Happened to me quite a few times that I simply didnt see a tattoo.
Slavetats is a bit limited in speed with a lot of of overlay slots. It needs to check every slot if there is something or not so if you like me have a lot of slots it needs a moment. Just making a estimation by memory mine takes like 5 seconds with 40 slots or something like that.
Since you didnt have rtats before ensure you've set it up properly and understand how its category system works. You either edit the settings.json manually or assign tattoo's in the mcm to categories that reflect different body parts.
There can only be one tattoo per category, if a category (area) of rape tats is already covered with a tattoo of those you assigned to it, it will pick a different area, so yes and no for your question. If you have all possible categories already covered, it cant add anymore, but otherwise it wont be the cause for a tattoo to not get placed.
Rtats has a lot of categories, specifically if you make use of its misc categories. If you dont make use of them I recommend my tattoo packs rtats section for a recommendation on a system to make good use of those additional categories that are not meant for any specific body slot by default.

😂 My request would be to make it much more expensive. It would be good to have a setting in the MCM  that allows players to modify the amount according to their play style. Ideally, I would love to have it connected to character level, and adjustable. So, set the price to 10, 20, ... 50... 100 per character level, so it scales with character growth. I find that it is harder to keep a balance with costs as the character gets to be a higher level and can make so much more money.

+1 to the idea of an adjustable scaling price, I figured out (after a very long headache, due to how little I understand of modding) how to adjust the cost but a scaling system would be be amazing.

at the minute i have the cost of removal set to 2000, when combining that with licences and daily requirements like food and drink for ineeds the cost becomes comfortable through a playthrough, at the beginning its admittedly very tough, but even into late game, the cost of gear up keep means sometimes that 2000 is difficult to reach.

I really like this mod, It fills a much needed gameplay niche. I would LOVE options to increase the cost/scale it based on number of tattoos/remove 1 random vs remove all.

(Also your tattoo packs are great - how do i get my tats to show up like in your screenshots? I can't figure out the settings to make it happen. Everything always looks much thicker and painted)

There is no settings you just apply the tattoo with slavetats "Everything always looks much thicker and painted" isnt really telling me much, send me a screenshot (in my slavetats thread or dm), but if it looks different you probably either use the wrong body or have a lower quality skin texture.

Nevermind! I ended up finding them, they were just specific tattoos i was looking for

Can this mod remove tattoos, that were applied by another mod and are not removable in slavetats mcm menu (grayed out)? The mod in question does not work properly anymore and does not remove the tattoo.

sorry for the late reply I missed the notification I guess. The tats you refer to are from my riekling pack they use the same kinda styling as my orc tats.

I'm not seeing the dialogue for removing tattoos, even if I load the mod last, but I'm playing with Requiem that make changes to most NPCs in the game. Can I add the dialgoue manually to the court wizards?

Got a few mods that add additional dialoge, no problem. If you have some mod that fucks over the npc's and exchanges them for something else then it would make sense for the dialoge to be gone.
You can add the dialoge to any npc with xedit, just have to add the npc as condition like the others for each dialogue option. If its from a mod that mod needs to be made a master for the esp
