If you have Origin open you can right click on the sims 4 icon and it has a menu for 'move game'.

The game is already on disk D, but the mods, saves, ecec remain in C. I moved them to D, but in game it does not read them

Since the game likes to use the default C:/Username/My Documents/Electronic Arts/The Sims 4

 

You'll have to do this to change that folder location:

You may either move your whole home folder to D or create a link (mklink) for the folder "Electronics Art" or "The Sims 4" to D.
