Hasn't been done yet and I suspect it's not really even possible.

Vaginas are just applied like an overlay or skin detail and are external only.

Sims are all basically hollow frames wearing a skin overlay/outfit.

 

If you have Get Together EP go visit the Haunted House and find Mimsi the ghost.

Her husband Bernard might beat you to it though.

Shift Click on him, select Wicked > Disable for Autonomy.

 

Or find a female ghost in your town as those are the only ones I know to be transparent.

As one can override the top/bottom body one should be able to duplicate part of the mesh and invert normals using the same UV map. Removing some outer mesh faces should allow see-through.

As there are no organs one would need to create them as well and invert normals as well.

The upper/lower body split may cause some headaches as see-through may be needed in both parts.

 

Quite easy to create, likely violating copyright of EA or other mesh/body/organs creators BUT which animation takes care where the dick is inserted and about the angle? Aanimaters would need to support it and also animate the cum mesh accordingly inside.

 

Sometimes one may enable the 'fantasy' switch and imagine how it would look, feel, smell if it was real.

Actually a TS4 bug already allows to look into the sims. It's annoying when feet, hands or part of the breast mesh are missing. And it looks like this:

PS: The red plumbob in front of the head is my move-a-sim overlay window to position the sim with the mouse (to be released).
