Arm glitch when my sims hold objects?

This weird arm glitch is keep happening whenever my sim holds something (book or plate etc) and I have no idea what's causing it.

 

Any ideas?

Probably broken CC (my guess would be the top). Does it happen with non-CC clothing as well?

I've even had bad cc hairstyles cause arm glitches but not quite that severe.

I found it.. weirdly enough it was Nandong head default? No idea why ahah must be Bcos WW comes with a default tongue now I guess.
