I have seen her before and not from the mod space octopus mentioned. Try searching for "Elf follower" on the Nexus. There are probably others that look just like her.

The name of that follower is Stephanie, and she's still work in progress. Mobear makes wonderful followers. His Serana and Cailin, in my personal opinion. are stunning amazing.

It does appear that we won't be able to have Stephanie anytime soon... however, I did discover that Mobear is still active on his website! For fairness reasons, I think it's more correct you ask him to re-upload the mod (there is a form at the bottom of the web page) or his authorization to share it.
