BrittPinkieSims (?) made a short guide on how she created her Fashion career and there are other guides on how to do it. I fI had any coding knowledge whatsoever I'd give it a go but I really don't :c

I have been working on converting my old Hitman career I did for another sims game to work with TS4, maybe when I'm done I can look into doing an adult one, once I'm done. unless an other does it first.

Think you would need to have it done in a way so it keeps being interesting, meaning active careers with advancement based on performances. Whims attached to it and things like that, think if only possibility is to charge money for wicked woohoo, i think it would grow dull really fast and uninteresting.

Please note that both of the adult career mods have issues. 

Career Erotica is pretty much dead and doesn't function properly with the current WW build.

 WickedJobs was one of the many mods broken by the C&D update and will need updated when the creator eventually returns.

Not a problem anymore. An unofficial fix has been created for WickedJobs.

Just download the 0.2 fixes that have been posted in the comment thread. (The TV porn channel still doesn't work though.)
