On every single save if I load them directly from main menu.
However..

>Start skyrim
>Main menu
>Start new game
>New game loads
>Okay character
>Load old game ( or last save)
>Loading screen.
>Loading screen...

>Loaded!!
>Play for 5 hours with no issues.

>Exit skyrim
>come back tommarow and repeat process

Wtf ?

Anyone know why i cant directly open my saves, but if i make a new game and readload the save over it it ends up working fine ?

This is actually very common. I can't remember the last time I even bothered trying to load my most recent save straight from the main menu because a CTD at the moment it finishes seems almost guaranteed with modded games once you get far enough into a playthrough. No one has ever been able to work out exactly why, as far as I know - whether it's down to number of mods, specific mods, size of save file... Who knows?

The workaround, once it starts happening, is to load your most recent save from an already loaded game. I use Live Another Life, and I keep a save from the beginning of a playthrough in the LAL cell once all my MCMs are configured and before I've chosen my start. I load that first, then my most recent save. A save that early always loads fine for me. Once you get into the habit it's not too bad. I've seen other people suggest opening the console on the main menu and typing in:

coc qasmoke

to get into the test cell, and loading from there. Either way, it's a good habit to get into.

To go along with what Dreamweaver said, it's a problem that you're going to have to get used to. Way I understand it is, you've got too many script heavy mods all trying to load at once and if the size of the cell you're loading into is very larger(outdoors, for example) then it becomes too much for the game to load fast enough and it crashes.

Dreamweaver mentioned: coc qasmoke :and I couldn't agree more, I've never had issues loading a save after using that command from the main menu.

You can also try saving your game in a small interior location. For me, I have an unmodded Breezehome and I try to save there as much as I can when I'm done playing. If I stand next to the main door without any other actors in the picture, and only turn the camera enough to open whiterun, I can load that save 8-9/10 times without crashing. If you're in doubt though, use the coc qasmoke command, that works pretty much every time.

Yeah, i have been doing it for quite a while and was wondering if anyone had a fix, I mean its only a couple extra seconds so its liveable. I tried that save game tool and i still get the CTD; oh well. It could be worse !
