I used to know what caused this but that ship has sailed.

I've not touched Skyrim for a few months so I'm a bit rusty. I've had this problem more than once in the past and fixed it.

 

There is a follower on the other side of the pillar and this is coming out of her head, eyes or brows.

This is a different follower and the phenomenon is barely noticeable. Still the head region but hard to tell.

It reminds me of the old bug caused by HDT. It was related to the NPC's weight. 

 

To check :

in the console If you can find it, select the NPC and type setnpcweight 100

 

If the problem is fixed, you can try increasing the weight in the  Follower ESP

I would say these are 2 different problems. The first one has many causes:

  1. GPU memory corruption or display driver bug.
  2. Mesh corruption or other mesh issues.
  3. Animation and morph issues.
  4. CBPC and SMP physics.

The eyebrow z-fighting in the 2nd image I get with male beards as well and it started after I rebuilt everyone's facegen data. Not bothered to look into fixing it yet, it's only on a select few NPCs - the elf in The Drunken Huntsman is one example. Brows and beards are meshes too so the most likely cause here is that the brow mesh slightly overlaps with the head mesh. The curious thing about my beard issue is that I'm fairly certain I have no custom male head or beard meshes installed; but like I said, I haven't looked into it yet.

It's an HPH issue.

In the top image she had an HPH but the esp was pointing at the vanilla head. Re-pathed the esp entry and done.

 

Bottom image a bit more complicated. Problem was eyes in Head Parts PNAM was pointing to a head part that didn't exist in the left pane of XEdit. Deleted that reference which solved the original problem but turned her head into a potato head and her hair didn't look right.

Looked at it in CK and CK complained about scalp. Investigated that and she had HG hair using KS Hairdo's scalp. Re-pathed in the esp and CK message gone but still got the head.

HPH Predator SMP ? If that's the case, try removing the SMP string from the head.

If you're using SMP hair, it's likely to conflict with Virtual Head

Don't use SMP.

Eyebrow mesh being used is for vanilla head use something like this instead.

 

I got my wires crossed here. The eyes were part of the problem in the top image. The problem here was indeed the brows and it was indeed EMB which I spotted way down at the bottom of the description page on Nexus. Re-pathed to EMB and that problem went and left me with the potato head.
