That looks to be one of the Devious Devices Contraptions. It's part of the main DD package.

 

LE here, SE here, and NG for SE here (the most up-to-date version if you're on SE.)

 

I haven't played around with contraptions very much myself, so I'm not certain which one it is, or how best to trigger it in-game, but I know I've had my character locked in it a few times.

 

(I use DD NG, so it's possible it's an addition for that version, and not in the LE or standard SE package.)
