I cooked something up just for demonstration purposes. I prefer my belle pasty so I just reverted it back to the original. Had all sorts of issues that the screenshot doesn't show, and would have taken more work to be presentable beyond the screenshot anyways.

Not exactly sure what the cause is, but I've seen this happen with Miyabi in the past.
Usually of no consequence because it's exclusive to the menus, and once in the world or gameplay, she would be visible again.
Also, never really persistent, just something that might happen occasionally.
