Ugly Generic Follower

Hello, I've been thinking lately quite some time about having an REALLY ugly, old follower around, with nasty homeless person kind of look.  Though unfortunately i have absolutely zero idea how to create a follower, or change face of some already existing npc. I was wondering if it wouldnt be too much to ask - but could someone be so nice as to create something like that and share it here? I was thinking about something like that:
