Do you know a game with real adult content mods who is not TES/FALLOUT/SIMS?

I could recommend starbound but I think looking at pixels is not what you need. On the Internet there are a large number of games on such topics, most of all the Japanese succeeded in this. Something like Rapelay you might like.

if you don't care about old graphics, there is Dickplomacy/SOTT/Warphilia/Warwolf for warband, also a dance with rogues for Neverwinter Nights(text only, at least the first one), that is for mods, i prefer going to porn-games or h-games if i want more porn than gameplay, mods for more gameplay than porn, but thats your taste, at last, there is Rimworld, and Darkest dungeon, good luck in your search anyway!

Conan Exiles is the only one I can think of. No a mod but there is plenty of adult games around. Most of them is from Japanese developer like Honey Select or custom maid 3D. Don't bother with Patreon games by western developer unless the only thing you want is sex scenes to fap to because most of them is still early in development and have little to no actual gameplay.  It is either that or 1000 renpy visual novel use the same DAZ model over and over.
