Randomly missing meshes, textures appearing on wrong objects, help please.

Hi everyone. I'm having a rather complex issue and honestly have no idea where to even start troubleshooting this. After some time of playing, the game just starts going totally crazy on me. Textures appear on things they don't belong to (hand texture on a crate), often even switch to a different one as I look at them, meshes randomly disappear, the works. http://imgur.com/a/B09oJ here's a few examples. I know for a fact that nothing is actually missing, as when I restart the game, everything is fine again for a while. Any help would be appreciated. I'm staying below 139 esps.

Usually I have this issues, when my game runs out of memory. Just save, quit and restart, and your good to go for a couple of minutes/hours.

And I'd add - if save, quit and restart works, it means you're over the number of plugins you can handle and should lower it
