Do you use some kind of skin textures for males, like... favoured soul meshes or something? Hope it's the right name^^ If so, SOS should overwrite these kinds of mods. At least, that's how it worked when I had the same problem^^ If you use MO, make sure SOS is below these mods on the left panel, you can also check what mods are overwriting/getting overwritten by SOS.

Usually this occurs because someone installs better males, then they don't uninstall Better Males completely before installing SOS. The other possibility is that you have a male NPC replacer that has a body included, so you should search here or on Nexus about the NPC replacer mod to see if there is an SOS version. Basically - if it's just (several, most, or all of) the NPCs (and not the PC) that have the double-schlong - it's an NPC replacer problem. If it's both NPC AND the PC, then you need to uninstall the other body mod completely (Better Males usual culprit).

As I mentioned above, you either have a male body mod installed (something like Better Males) in addition to SOS, or you are using an NPC replacer (Males of Skyrim, for example) that includes its own nude body which causes a double dong - the nude body included in the replacer and SOS.
