some monster and followers don't aggro

Hey i have a weird issue who only occur in exterior/outdoor

 

I have some monster and bandit who don't react to me, i must be very close to them or even try to interact with them to make them finally aggro

Same with my followers outdoors they don't fight, they watch me killing monster but if they are not targeted they will not react to the fight

 

Its totaly random sometime they will trigger and initiate but majority of the time they just stand with their weapon draw, some monster who aggro will just rush me and ignore my followers too

 

any idea someone ? :/

Appear you have a problem with your follower mod, but you not say what are you ussing.. try reinstall it... or un-install and re-install... and use LOOT after every operation...
