I use paint-net which is also free and similar to photoshop. It's what I used to make most of my tattoos as you can add your template .dds then drag and drop the next thing onto it's page and select open which will open it in a new tab beside the original the add layer one will place it on top of the template.

What I normally do is find a tattoo or something that I think will look good and grab it. Next load it into gimp2 and add a alpha channel then cut out everything that isn't needed either with the lasso tool of magic wand depending on close the colors all match if they all match too closely you will not be able to use the magic wand as it'll select too much stuff to be removed. Save it.

Next open paint-net and drop the template body .dds file onto the screen then drag and drop the modified file you just did and select open. Got to the last file and select the magic wand and then hold down the CTRL button and click what you want then keep clicking till you highlight everything for the item and there are no flickering spots left that are needed then use the cut option to cut it out and click on the template file and paste it onto the page and adjust and scale it to how you want it click save as to overwrite it. Have the body open in nifskope so that you can monitor how it will look in game. If it doesn't look right go back and undo it and then adjust it some more.

These ones here that I did awhile back are from a fallout 4 tattoo mod. Since they both use the same textures as skyrim all I had to do was delete out the fallout 4 body texture and then transfer it from that file to my skyrim UNP body.
