There was a glaring omission in this topic - an adult game aspect that is crucial for immersion:

 

creatures.

 

Including creatures/creature genitals into the game (something similar to Nasty Critters I guess) is one of the most fundamental aspects of realistic/adult gameplay.

 

The only question is: will there be supermutants with tits?

one problem with the vault being a cryo brothel that you'd know if you read the terminals: most of the people in those died, i'd put my money on heart failure, either heart attack from stress/shock of being frozen against their will, or heart simply bursting as it's in the warmest part of the body, therefore the last to freeze and the most heavily effected by the increasing blood pressure

 

not to say it's not an awesome idea, but the point was to be lore friendly was it not?

 

that said i guess it's not impossible the raiders have surgeons that can perform heart transplants as the people thaw, some medical texts have to have survived i'm sure

Actually, it is explained later why they actually all died. Kellogs had orders to disable life support for all other pods (I think he say "Why don't we refreeze the others ?" or something similar in the Dangerous Minds/Shattered cutscenes). And, although we can't say why he had these orders, we are sure that no one freezed in Vault 111 died from "natural" ways.

How about a quest tie in for Kellog and Covenant.
It would be cool to have the player captured by Kellog in the final confrontation. Make them work for ffreedom before the big fight. 
 

Covenant is cruel, this could also be a captured and abused situation for make and female players. Think the new BSG when the Pegasus had a captured Psylon... they could incorrectly, or correctly assume the pc is a psylon and hence, not worthy of rights and dignity.

Also, in Fallout 2, you meet a supermutant named... francis... I think. One of the options for getting past him is to be his gimp for the night. When done, you end up with a ball gag in your inventory and some choice dialog over your head.
Hell Fallout 2 is an inspiration in itself for sex mods. I like most of what was done in NV... except it was pretty broken.

With the new settlement building, you could also have raider raids go horribly wrong for the settlement and have them as roving rape gangs instead. Or you could be the agressor
Build a slave town
build a Brothel
Build your own custom Sky Casino. etc...

In short, I am looking forward to this. I had an idea for Skyrim that involved either the Stormcloaks or the Imperials capturing the PC  (Via Rikka for cloaks) and keeping them in a dungeon to abuse. the idea for the cloaks is that they capture Hadvar before you. Hadvar escapes with Rikka through your actions and... errr... distraction. Rikka gets willing captured to infiltrate and help you escape. in the end, you both escape and put the ordeal behind you as best you can (Female/Gay only of course).

personaly i hope for some better girl on girl stuff not just the same animations but with a strapon. i realy like to romance ideas. i had many sitiuation were i was like oh Piper i could kiss you right now. it could also be intersting if romance parters would react on you being faitful or not like a few das ago while romancing Piper and having her as my companion i fucked Magnolia and she didnt even care. she should react by either saying "how could you" or by "that was fun"  or somthing like that depending on relationship settings.
