Is there a way to make Devious Devices more tasteful?

I like DD in concept, and certain items do seem appropriate for Skyrim's setting. Yokes, Wrist Irons, Ankle Shackles, etc.

But then you have... all the other stuff. Hobble Dresses, Ball Gags, Straight Jackets, leather BDSM suits, gas masks. I can maybe MAYBE see some rich Imperial noble having *some* of this shit in their personal BDSM dungeon in their estate or whatever. But getting equipped with this shit by random ass bandits and commoners just takes me out of it. Is there a way to disable most of this stuff and just have the basic medieval-esque restraints?

Well, as the DD framework itself doesn't equip anything on you, you're free to choose what you wear.

If you're looking for something like Cursed Loot, but want to choose what gets equipped, you might want to try Devious Curses NG (assuming that you're on AE)

You can edit the json file that defines possible sets, and have an exclusion list with all the stuff you can't abide.
