My female sims never undress completely in Cas even under the tattoo category, they keep a big ea bra on.

A bra in CAS typically means that you have a so-called "default replacement" in your Mods folder which interferes with WW. These files usually have a "default" or "DR" in their filename and most often come with custom body parts, sometimes also underwear and the like. In your case, it's most likely a female upper body (i.e. breasts).

 

If you find any DR file in your Mods folder, remove it and remember to delete "localthumbcache.package", found in "Documents/Electronic Arts/The Sims 4", before launching the game again. Your Sims should be fully nude in CAS now.

If not, or if you can't find any DR files, you'll need to 50/50 through your Mods folder, starting with only WW (and, if you have it, MC Command Center) as your minimal setup. I’ve had this issue and the bra wasn’t labeled as default or replacement. Sometimes default skin replacements can do it too. The 50/50 method is (horribly tedious) the only way to really know.
