Under NG as a separate mod and overwriting it ❤️ You need the original Slave Tats, then Slave Tats NG, then my patch. If you have Lewd Marks or Slave Tats SPID, they need to be under it. See the image below. For me I could see Slave Tats MCM no problem.

No, not on new games, only on existing ones. Mods install their tattoo packs in "Data\Textures\Actors\Character\slavetats", a .json file which describes what tattoos the pack has, and one or more folders with the actual textures. If you didn't remove the .json file and the folders, this would explain why SlaveTats still finds the textures, even though you disabled the mods (guess you mean the .esp with that). But this doesn't explain, why it applies the tattoos in the first place, some scripts must be active, doing this.
