Usually it's caused from having 2 different female body tops installed or 2 different defaults installed at the same time.

 

Female Tops/Nipples generally use a mesh that makes the bumps along with a texture/overlay that adds the colors and details.

When these don't match, like when using different Female Tops, then they don't align correctly because different creators have different shapes.

Sometimes you'll even see 2 different bumps on the breasts too. 

 

I'd look for anything like default female tops/body parts you have installed and remove them.

CMARS & Better Bodies are 2 defaults I know of but they should be compatible if you only use one or the other.
