I've checked "Another cunt" and it looks like I have to remove male sim's penis in order for it to work.

If you want an M-Herm, you will have to use Noir and Dark's Futanari Penis for males (no testicles variant).

M-Herms are the only kind that exist in mainstream American movies and television (see 'The Prophecy' movie or nearly any adult themed sci-fi show that airs on Sci-Fi/SYFY which features humanoid aliens for examples).
