Like in this examples, the first one it shows your penis correctly. But in the second example the penis get bugged with the trousers color skin. How can i fix the issue of the second photo?

Randomly till i get with the key? It's not UV, it's texture. The texture must match the UV. I can't give any advice about Sims4 because I don't have it. But meshes, textures and UVs are no different in Sims4 than in other games. This texture suggests that the UV is correct (if you look at the shape). So you need the right texture (skin texture).
