That is correct. For any nipple piercing to work properly and not look off in any way, you require a matching nude top (which is applied via WW's Body Selector) and clothes (unless you want your Sim to be nude all the time).

 

With your current setup, you should be able to use piercings made for the default (or "vanilla") Maxis body just fine - however, I have only used those in combination with Luumia's TIT-E nude top, so I'm not 100 percent sure about visual consistency.

 

Most piercings will show up over clothing because of layer sorting in CAS, so go for custom clothing with the nipples visible, or else it'll look strange.

 

Piercings for other nude tops usually tell you which one they require exactly, like EVE v6 or BetterBody (BB). There are a lot of clothes for those body parts, too, so creating some outfits with nipple piercings shouldn't be a problem at all.

 

As for conflicts: "No default replacements" is the only rule here. WW already replaces the game's defaults, so use non-defaults and the Body Selector for nude tops etc.

CC like clothes or piercings also fall under this rule, but other than that they rarely cause any issues. If some don't show up in CAS, they're simply broken, so remove them from your Mods folder and go on like nothing ever happened.

The whole thing with the "defaults" is about the files in your Mods folder. Take a look at EVE as an example:

Inside the downloaded .zip you will find a folder containing all the files and within that two sub-folders named "Defaults" and "Non Defaults". This is where the rule to not use default replacements (like the contents of the "Defaults" folder) comes into play.

 

You are correct about having to put those files into the Mods folder in "Documents/Electronic Arts/The Sims 4", but you can create sub-folders there to organize everything better (like one for clothing items, one for WW animations and so on). The important rule here is to never place .ts4script files deeper than one level of sub-folders, because they won't be read by the game otherwise. "Mods/WickedWhims" works, "Mods/Sex/WickedWhims" does not.
