Assuming you have already copied the ZIP file to your Addons folder, it is simple: Open the Customizer (key F12) and on the Dress tab (the first) you scroll down to the Waist category. Use the dropdown menu to select "Gymnastic Leotard".

 

If you installed any ActiveMod textures for this addon, they will also appear in the same dropdown menu for category Waist.

the Dress Category (also known as Dress Zone) of any item has to be set by the creator and sometimes an item doesn't appear where you would perhaps expect it.
