Does anyone know how I can change the gender of a sims without it changing its appearance?

Is there a mod that solves that? Or some program that i can use to just change the gender of that sim and keep their clothes?

Please help :( and sorry for maybe a stupid doubt

The game itself can actually do that, using the advanced gender settings in CAS.

Click on the three little dots right under the "male" and "female" symbols to bring them up, and you will see four sets of options (all of which are mutually exclusive or "either-this-or-that" settings).

The first one just changes the Sim's physical appearance. Face and hair will change, breasts will appear/disappear, but outfits will stay the same.

The second one just toggles, which outfits you'll see in CAS for this Sim by default. "Male" and "female" are just filters like "swimwear", set in the outfit selection screen by the game for you. You can change those as you like anyway, so this is really just a preset that can be annoying, if it is "wrong" for you (e.g. you want a female Sim to dress like a guy, but the game only shows you female outfits at first). The downside here is that female Sims wearing male clothes will look a bit "off" at times.

Pregnancy options (third menu item from the top) are self-explanatory. Use these to create trans/futa Sims that actually "work" as intended in-game.

The last set of options toggles whether a Sim can use a toilet standing in vanilla TS4, and whether they have a penis with WW. Use this together with the pregnancy options for consistency in your game (i.e. Sims that can impregnate others should probably have a penis).

I have tried but in this case, the breasts only appear / disappear when I change gender itself, from male to female and vice versa.

In my case, in the physical section only changes the face and the texture of the sim but still maintains the man's chest even though the body is configured as female, so when he wears a swimsuit the chest is still the man's so it looks flat.

(All this is for a pre-made sim that in the post says that he is female but in my game he is male)

Okay, so apparently, the breasts on my initially female Sim had just become too small to notice, when I tested this. With another, male Sim it was the same as with your Sim this time, as was with his female roommate. It really does change just the physical frame and not the body parts, as can also be seen in WW's Body Selector (where the available categories remain the same, but selections are reset to their defaults).

However, and as one might have expected, there is a mod to copy any given outfit, created by Scumbumbo himself. Despite being from back in 2018, it still works like a charm, and unlike MC Command Center and its Dresser module, it doesn't care about pesky little details like a Sim's gender.

You can download the mod here: https://modthesims.info/d/541770/outfit-tools-copy-any-outfit-v4.html

What you'll want to do: Pretty much follow the steps to fix messed up outfits by using the "outfit.backup" and "outfit.restore" commands (starting with step 3), but instead of switching saves (because these steps are about fixing some mess created by some patch a good two years ago), you'll have to switch your Sim's gender in CAS. Just click the female icon and don't mind everything being completely messed up for the moment, as once you restore the outfits, everything will look just like it did again.

My male test Sim even got his/her beard back until I went into CAS to check all the outfits.

There are also commands to copy outfits from one Sim to another and between different outfit categories, so if you're interested in doing that as well, take a few minutes and read the complete instructions over at MTS. It's really a great mod - especially since it's just a little script file.
