It is 100% not an overlay. Because for this you need a suitable mesh (morph from OS is enough) and you need at least a "neutral" normal map.
So it can be a mixture of Morphs and TextureSet - not a complicated thing.

You can create morphs in OS. Open OS, load the Project (your Reference), click on Slider-> New Slider. Enter a name for this slider and click on Ok.

Activate the slider and make your reshapes. To be sure, it's not only a local morph, click on Slider and export your new slider as bsd.

Save your project and close OS.

Find your sliderset of your reference (xml/osd file) and open the file (Notepad++, etc.) and check your new slider setting. If it local, change it and set the path and file to your exported bsd.

For a neutral normal map (without nipples) you can in OS export this slider as Obj. With xNormals and this obj you can create a new normal map.

In Gimp or Photoshop you can use this normal map as blend layer for your original normal map. Maybe you must switch the RGB channels of the normal map in the  blend layer.
