There is a tool to copy/paste the original/vanilla NPCs' faces. NPC Nif Merge. https://www.nexusmods.com/skyrim/mods/40494? You just need to find the original Serena's head meshes and textures. Because NPC Nif Merge is for Skyrim LE, you need to convert the nif/texture files from SE version to LE. For that reason, you probably need such tools like Cathedral Assets Optimizer, and/or SSE NIF Optimizer. If you don't know how to find an NPC's head meshes/textures, google it. If you don't know how to use Cathedral Assets Optimizer/SSE NIF Optimizer, google for tutorials.

I always suggest to make a "Clean Save". If you have a "save" that has no problem, with main/required mods, and working properly shouldn't cause any problem while you run Cozy Inn. If you have corrupted/unstable mods, and/or bad saves, Cozy Inn might not work from the beginning. So, be careful. First, be sure that you have a working, (not causing any trouble, not crashing...) save. You can try to use that one. If you concern about achieved levels, you can download and use a "Save Game". I know nothing about Ceraph follower. I don't know how it works. You may ask/request EvilReFlex to create TS Followers with TNG add-on. He knows better. My knowledge is very limited. In the games, there are only two sexes: Male and female for PC/NPC. Not transsexual. Maybe, transsexual people would like to use a body shape as they have by this experimental project. There is a mod in this experimental project, named "BodyChange". The cool part of it is it adds the transsexual body in a different slot. It doesn't replace your main body. Also, it effects only the PC, not any other NPCs. Maybe, you've noticed. If you replace femalebody meshes and textures in their folders, it replaces all female bodies' meshes and textures (PC's and NPCs'...). I would like some good/experienced modders, who would like to make some update(s) as your request. Wouldn't it be nice? For some specific settings, SexLab and SOS are fundamentals. I don't know if it's possible to create the same kind settings via Ostim to make a transsexual like NPC/PC. Cheers...
