I got this image from skyrim.jpn.org, where lots of the content posted is custom/private use/etc, so it might not be available anywhere. But just asking in case anyone's seen it before. Checked the usual places already (eskrim, eihei, mitakusaner, dragonporn, and of course nexus). Any help would be appreciated.
