Here's a step-by-step guide.

No slavetats (this works for other games as well):

Step 1: Identify the actor you wish to create the tattoo for.

Step 2: Identify the body\face texture of said actor.

Step 3: Translate the texture into a file format your particular brand of image editor (usually photoshop or gimp) can read.

Step 4: Create the tattoo over the skin texture. This is usually done via applying brushes or overlays to the desired sections of the texture.

Step 5: Save the new texture.

Step 6: Translate the new texture to the format of the game.

Step 7: Replace the old texture with the new one.

For slavetats:

Step 1: Locate any texture for the section of the body you wish to create the tattoo for.

Step 2: Translate the texture into a file format your particular brand of image editor (usually photoshop or gimp) can read.

Step 3: Create the tattoo over the skin texture in an overlay. This is usually done via applying brushes or overlays to the desired sections of the texture.

Step 4: Discard the body texture. Save the backgroundless overlay.

Step 5: Convert the new texture into the format of the game.

Step 6: Place the new texture in one of the slavetats subfolders.

Step 7: Create a .json file to tell slavetats that your texture exists. Alternatively, modify a .json file to include your texture. NOTE: .json files can be opened by notepad++ and reverse-engineered very easily.

ALTERNATIVE: Instead of steps 6-7, switch one of the tattoos that have a MATCHING BODY LOCATION of a different mod with the new texture.
