Missing mesh. She's using an armor or clothing that the game can't find a mesh for, so it puts up an exclamation mark instead.

 

She normally wears black robes, right? Do you see the same thing on necromancers, warlocks, etc?

 

EDIT: Monk robes, not black robes. I think Hiemskr wears those (but you might not notice since he's male, and it might be a female-only problem).

You could try spawning some and putting them on to double-check (if your character is female, that is). The item code is BACF3.
