That's a texture replacer. You need a compatible nude bodymesh for it to work correctly. According to the mod description it's intended to use for CBBE, UNPC, UNPB and 7B. Just check out what body most fits your tastes, install that one first and then any texturepack on top of that.

The underwear is part of the mesh and texture of the vanilla body. If you removed these without replacing them then you would see nothing at all, your character would become invisible. The only way to remove underwear is to replace the mesh and texture completely.

 

If you are new to modding and want the quickest way to do this then you can:

Install NMM (Nexus Mod Manager) - http://www.nexusmods.com/skyrim/mods/modmanager/?

Use NMM to install:

CBBE body - http://www.nexusmods.com/skyrim/mods/2666/?

 

There's many more options if you're willing to search through the available mods but that's the simplest way I can think of to remove underwear from females.
