If your using 3DS Max, you can use the Skin Wrap Modifier to your custom mesh. Make sure you have an imported Skyrim model in the scene, (in this case the SOS body for the body and clothes) so you can add it as a reference object. Tick apply weight to all points, then click on convert to skin. You can apply this method to the head and hair models but you would need to import a vanilla head mesh and a long hair model.

As for texture, you need a DDS plugin for your graphics program in order to have working textures ingame.

One thing to note is, your custom head models can't do facial expressions, you'll have to make morphs for it. I have no idea to do that though.

Here's a good video tutorial to get you started with skinning your mesh for Skyrim. It'll show how to use skinwrap modifier. I learned alot from this, check out his tutorial series as well, you'll find it very useful.

I strongly encourage making backups at whichever the case, and also experiment. You'll learn new and neat tricks. ;)
