Idea 01: In Skyrim NPCs change their dialogue to you depending on whether they like or dislike you. So if you have sex with too many people too frequently then you should get a status called "slut" or "playboy" for a few days, in which NPCs of opposite sex will talk to you like their friends (also you gain speech bonus against them), while NPCs of same sex will talk to you with disliked tone (and you get speech penalty against them). If you don't act like a slut / playboy for a few days the effect will wear off, indicating they gradually forget about it. Idea 02: When you have sex in public it should be considered a crime. NPCs should react like they see a crime and call for help. So if you want to have sex you should consider discretion in term of time and place. Idea 03: In Skyrim if you got diseased, NPCs will say you look sick. If you are covered in cum, or got high arousal, then I think NPCs should also say the same to you, because you know, "you don't look so good..."
