CTD on leaving caves?

If I go into a cave, there's about a 50% chance that it's impossible for me to leave. During the load to get back to the open world, the game crashes so I'm basically trapped in. Sometimes I won't have any problems but I'm not sure why? Does anybody know what's going on with that?
